How Long Does the Virginia Medicaid Caregiver Approval Process Take?

Approval to be paid as a family caregiver through Virginia Medicaid takes up to around 12 weeks.

The timeline is shorter when Medicaid is already active and documents are submitted promptly.

Medicaid Approval

If Medicaid is not already active, Virginia has up to:

  • 45 days to process a standard application
  • If the person is under 65 years old but has blindness or is disabled, the state has up to 90 days to process the application because they need extra time for a disability determination (learn more: How Does Virginia Medicaid Make a Disability Determination?)

CCC+ Home Assessment

Long-term care services (CCC+) must be requested before the state schedules a home assessment. If applying for Medicaid for the first time, CCC+ can be requested on the initial application. Otherwise, if Medicaid was already active, CCC+ can be applied for when in-home care is needed.

During the assessment visit, a nurse determines whether the person qualifies for in-home care.

Delays occur when:

  • A CCC+ assessment was not requested when needed
  • Contact information is incorrect
  • Assessment phone calls are missed

Approval of Care Hours

If the person qualifies for CCC+, Virginia sets the number of hours per week a caregiver can be paid to provide care.

Caregiver Enrollment and Payroll Setup

After care hours are approved, the caregiver must complete hiring and payroll setup before payment can begin. This includes:

This stage takes 1 to 2 weeks when paperwork is completed promptly.

When the First Paycheck Arrives

Payment begins after:

  • Care hours are approved
  • Hiring paperwork is complete
  • Hours are recorded using the required Medicaid time tracking app (EVV)

Common Questions

Can I Get Paid by Medicaid While Waiting for Approval?

No. Caregiver pay does not begin until:

  • Medicaid is active
  • CCC+ care hours are approved
  • Hiring paperwork, background check, and payroll setup are complete

Hours worked before approval are not paid.

Can I Speed Up the Approval Process?

Virginia's 45-day (standard) and 90-day (disability-related) limits are maximum processing windows. Approval can happen sooner if the application is complete and verified.

In serious medical situations, parts of the process can move faster. Virginia can prioritize an application when a delay would seriously jeopardize the applicant's life or health. A physician must provide written documentation confirming the medical urgency.
