How to Choose a Services Facilitator in Virginia

Families using Consumer-Directed care under the CCC Plus Waiver must work with a Services Facilitator.

The Services Facilitator:

  • Conducts the in-home assessment
  • Develops the care plan — a written document listing approved tasks and hours
  • Submits the request for approved care hours to the member's CCC Plus health plan

How to Find a Services Facilitator

Services Facilitators must be enrolled Medicaid providers with the Virginia Department of Medical Assistance Services (DMAS). Families can search using the Virginia Medicaid Provider Directory.

Search for: Consumer-Directed Services Facilitator

Then filter results by city or county. Contact providers to confirm they accept new Consumer-Directed cases.
