What Documents Do You Need to Apply for Medicaid Paid Caregiving in Virginia?

To be paid as a family caregiver under Virginia Medicaid's CCC Plus Waiver, documentation is required in three stages:

• Medicaid eligibility approval
• Home screening to determine nursing facility level of care
• Caregiver enrollment and payroll forms

1. Documents Required for Medicaid Eligibility Approval

If the person receiving care does not already have Virginia Medicaid, eligibility documentation is required.

Identity and Citizenship / Green Card

• Virginia driver's license or state ID
• If Citizen: Birth certificate or U.S. passport
• If Lawful Permanent Resident: Green Card
• Social Security number

Virginia Residency

• Utility bill
• Lease agreement
• Mortgage statement
• Official mail showing Virginia address

Income (Most Recent Month)

• Social Security award letter
• Pension statements
• Retirement distributions
• VA benefits letter
• Annuity payments
• Any recurring monthly income

Assets

Virginia Medicaid has a $2,000 asset limit for an individual applying for long-term care coverage under the CCC Plus Waiver. Provide:

• Complete bank statements for the most recent 3 months (include all pages)
• Savings account statements
• Investment account statements
• Life insurance cash value documentation (if applicable)
• Deeds for additional property

Virginia reviews financial activity for the past 60 months. This 5-year lookback examines transfers of money or property during that time.

Applicants must submit Appendix D: ABD, LTC Application along with the standard Virginia Medicaid application.

2. Documents Required for CCC+ In-Home Care Approval

After Medicaid eligibility approval, a Medicaid home screening is scheduled. The following records must be available during the screening:

• List of current medications
• Names and contact information for doctors
• Recent hospital discharge paperwork (if applicable)
• Documentation of medical diagnoses
• Physical or occupational therapy records
